Write during the session
Type key findings as you go, then spend two minutes at the end to complete. Patients are comfortable with their physio typing, especially if you explain what you are recording.
Use templates
Most visits fit a few patterns: first assessment, regular follow-up, discharge. A template with your usual headings means you fill gaps instead of starting from zero. In Clinidiary, treatment notes have templates for common problems and save as you type.
Short is fine if complete
A good note has findings, treatment given, the patient's response and the plan. It does not need long sentences. Complete is better than long.
Keep a few minutes between patients
Five minutes between patients to finish the note costs less than an hour at night. Build it into your session length.
Never leave notes for tomorrow
Memory fades overnight, and one day's pending notes become a week's. Finish each note the same day.
Notes during home visits
Home visit physios can write notes on the phone between visits; good software keeps a copy even when the network drops. See notes between home visits.
Notes are only one part of the paperwork a solo physio carries. See doing your own admin for the rest, and avoiding burnout for why your evenings matter.
Frequently asked questions
How long should a physio note take?
With a template, a few minutes for a follow-up and a little longer for a first assessment.
What must a physiotherapy note include?
Findings, treatment, the patient's response, advice and the plan, plus consent and any red flags.
Is it okay to type notes in front of the patient?
Yes. Explaining what you record usually reassures patients.
